Initial Appointment

Always bring your insurance card with you. We will copy it at your initial appointment and verify the information at each follow-up appointment.

A copy of the card will be made in order to bill your insurance. Your co-payment or coinsurance is due at the time of check-in for each visit. Please be prepared with cash, check, or credit card (American Express, Discover, MasterCard, or Visa).

Follow-up appointments

We will be available during regular business hours to schedule your appointments. When scheduling an appointment for a minor, please be sure to inform us whether a legal guardian will be accompanying the child to the visit.

In an effort to increase the efficiency of our providers, help control medical costs, and assure prompt scheduling of patients, PCA has implemented a Patient Appointment Policy. Patients must give PCA as much advance notice as possible if you cannot keep your appointment. If you cancel your appointment with less than 24-hour notice you will be charged a late cancellation fee. We realize that situations outside of your control do occur and each situation will be reviewed. As always, we appreciate your promptness for your appointment time.
